Why Do I Need a Summary Care Record?

Having your information stored in one place facilitates healthcare staff in treating you during emergencies or when your GP practice is closed. This consolidated information can significantly impact a doctor's decision-making process regarding your care, including which medications to prescribe.

Who Can Access My Summary Care Record?

Only healthcare staff involved in your care have access to your Summary Care Record, ensuring confidentiality and privacy.

How Do I Determine if I Have a Summary Care Record?

Currently, over half of England's population has a Summary Care Record. You can inquire about the availability of Summary Care Records by contacting our GP practice.

Is Having a Summary Care Record Compulsory?

No, participation in the scheme is voluntary. If you choose to opt out of having a Summary Care Record, you will need to complete a form and submit it to our surgery. You can obtain the opt-out form at our reception desk or by downloading it below.
